Learner's first initial: ___________________ General demographics: _____ male _____ female ______ approximate age Length of time participating in and circumstances surrounding their participation in adult education programs:
Personality Traits: Describe mannerisms, disposition, attitudes, etc.
Literacy habits/traits: Describe learner's accounts of past experiences with reading and writing. Describe present literacy levels as related by learner.
"Telling" quotes from the leaner: Include direct quotes from the learner that provide insight into his or her literacy needs. Elaborate on the quotes explaining what insights the quotes provide for you.
