Fill in this form for each literacy lesson taught with your case study learner. The lessons should start as soon as you have gathered enough assessment data to determine learner needs. The last lessons taught should reflect Teaching Goals you identified after an extended period of time working with this learner.

1) You may re-type this form to fit horizontally on your paper.
2) Date = the date in which you taught each literacy lesson
Concept = the reading/writing skill or strategy taught in the lesson
Teaching = how did you teach the skill or strategy (methods, materials, procedures)
Learning = what was the EXPECTED learner goal(s); what was the ACHIEVED learning
Evaluation = how did you determine/assess the learning
Summary = overall success of the lesson (what worked, what didn't, what you would do differently next time)
